Charity funding feedback

We were delighted to be able to support your team through charity funding, this short form helps us understand the impact of funding, improve how we support teams, and share stories with donors.

It should take around five minutes to complete. The charity team will review and be in touch with any follow up questions as well as arranging a time to visit the team for photos if appropriate.

Please note that anything contained in this feedback form may be shared internally and externally to help promote the impact our supporters have through projects like this in our hospitals.
